SLEEPY HOLLOW FOOTBALL CLUB/AYSO CROSS REGISTRATION

Every player and coach in the SHFC program must cross register with the AYSO 220 web site. Every parent should
register themselves first, and their players active in the SHFC program, specifically selecting the option for SHFC Registration.
It is most important to check off and sign the appropriate electronic consent form at the end of the registration process.
At the end of the registration process, the site requests payment of $17.50 per player to AYSO 220.
PLEASE DO NOT PAY THIS; IT WILL BE PAID BY THE SHFC ADMINISTRATION.
Below are the instructions. Pressing on the AYSO Tarrytown/Sleepy Hollow link will try to open
a new web page/tab (check your popup blocker!).
I. ACCOUNT CREATION
If you already have an AYSO Tarrytown/SH account, perhaps for younger players in the AYSO program,
skip directly to login to the site and scroll down to begin with step 2 - player registration.
Begin by clicking here to create a new account on the AYSOTARRYTOWN.COM site.

Provide parent name, email, a unique username and password and press CREATE ACCOUNT!
On the next page, enter all parent contact information and press CONTINUE.
Once you have completed the account details, you must identify yourself as a parent or guardian by pressing the FIRST selection “I am a Parent”. 
II. PLAYER REGISTRATION
If you have previously added your player, you can skip this and move to the step 3, program selection and select your player's name.
At this point, you will be prompted to “Add New Participant”.
You do not need to add anything other than player name and date of birth here and press CONTINUE.
III. PROGRAM SELECTION
After you have created a player, scroll down, and select “SHFC-Registration”, it will change to “SELECTED”,
after which you press CONTINUE.
On the next screen, DO NOT insert in any new details - scroll to the bottom, check the two check boxes
for “I ACCEPT THE WAIVER” and press CONTINUE.
IV. CONSENT FORM AND E-SIGNATURE
The final detail is to completely sign the ESign Consent Form.
Failure to completely sign will not allow us to finalize player registration.
Click on the green “Click Here to eSign Form” Button to continue.
You must verify the info, and scroll to the bottom, where you must click on the “I agree to electronic signature” and “I represent I am parent or legal guardian” and type your name. Then go to bottom and press “Continue to Review”.
You will be PRESENTED WITH THE SAME FORM AGAIN – SCROLL TO THE BOTTOM AND PRESS “SUBMIT SIGNED PLAYER REGISTRATION”
AT THIS POINT, YOUR CHILD’S CROSS REGISTRATION IS COMPLETE.
